Common causes
A cause is found in some men and not in others. Recognised causes include varicocele; hormonal problems (low testosterone, pituitary disorders); previous infection or inflammation; undescended testes in childhood; genetic factors in more severe cases; some medications and anabolic steroids; and lifestyle and environmental factors such as smoking, heavy alcohol use, obesity and prolonged heat exposure. Often no single cause is identified.
How it is evaluated
Semen quality varies naturally between samples, so a single result is a snapshot. Repeat testing is most useful when the first result was abnormal or borderline, to confirm the finding before acting on it. Depending on the results and history, a specialist may recommend hormone blood tests, examination for varicocele, a scrotal ultrasound, and in more severe cases genetic tests. The partner’s evaluation runs alongside, because the right treatment depends on both.
